Position Summary
The Community Health Worker (CHW) position works directly with people at risk for HIV, STIs and TB. This position will offer rapid HIV, Hepatitis C and Syphilis testing. The CHW will facilitate STI and TB testing with community partner agencies through formal and informal networks and will connect people living with HIV to HIV Case Management and medical care. The CHW will work as a Disease Intervention Specialist, conducting contact tracing, risk reduction education, assessment and referral services and health literacy education for people at risk or living with HIV, STIs and TB. The CHW will also provide community education to providers and other community partners to build capacity, help reduce HIV Stigma, and improve access to resources for clients in the community they work in.
Essential Functions and Responsibilities
Receive and respond to referral requests
Conduct rapid testing, provide syringe exchange services and other prevention interventions
Maintain up-to-date HIV knowledge; provide HIV education to clients and community partners
Conduct searches for community resources and housing
Assist the client in navigating through the health care system by providing information and advocacy and modeling appropriate communication
Work with clients to address barriers to medical care, behavioral health and other services
Assist clients in accessing pharmacy services needed to obtain medications
Transport clients to and from community resources and assist them in accessing community resources
Obtain Authorization for Release of Information between HIVA and Medical Provider when necessary
Provide direct one-on-one and phone care services
Facilitate client referrals to HIV Care Coordinators and PrEP navigators
Maintain up-to-date and accurate client records
Participate in necessary meetings
Assist with all necessary reports
Participate in required training
Comply with all applicable standards, guidelines, laws & administrative rules
Other duties as defined by Director
Required Qualifications
Two years of relevant experience or a Bachelor’s Degree in Public Health, Social Sciences, Social Work/Human Service, or any combination of education, training and experience demonstrating the necessary competencies.
CHW certification preferred
TB test required, will reimburse
Ability to pass agency criminal background check
Valid Oregon Driver’s License
Physical Demands
While performing the duties of this job, the employee is required
- Repetitive use of hands such as grasping, twisting/turning of wrists, and finger dexterity necessary to perform various clerical duties, such as typing and writing.
- Frequent use of arms such as pushing and pulling to open and close drawers and cabinets
- Ability to sit or stand for extended periods of time at an assigned station
- Ability to bend, stand, and reach to be able to transport clients and to occasionally lift and carry up to 25 pounds (harm reduction kits, naloxone kits, testing and education materials)
- Continuous clarity of vision at 20 inches or less for processing of paperwork
